WELCOME TO FOUNDATIONS PHYSIATRY
Foundations Physiatry was established in 2025 in Fort Collins, Colorado to provide thoughtful, rehabilitation-focused care for patients navigating complex neurologic, muscular, and connective tissue conditions.
Our founder, Dr. Casey Emigh M.D., has a clinical interest in brain injury and concussion, hypermobile Ehler’s Danlos Syndrome (EDS), and neuropathy; however, our services are designed to support a wide range of complex symptoms and diagnoses.
We care for the whole person, creating a supportive, non-judgmental space where patients feel heard, respected, and taken seriously. Treatment is individualized and may incorporate a range of approaches, including rehabilitative, procedural, integrative, and collaborative strategies, depending on your needs.
Conditions We Treat

Evaluation and rehabilitation-focused management of hypermobility-related disorders, including Hypermobility Spectrum Disorder (HSD) and Hypermobile Ehlers-Danlos Syndrome (hEDS). Care may address joint instability, chronic pain, and associated musculoskeletal and nerve-related conditions, with guidance on evaluation and coordinated care for overlapping symptoms such as fatigue, headaches, autonomic dysfunction, exercise intolerance, and nutritional deficiencies.

Traumatic brain injury care ranging from mild to severe, including concussion and post-concussive symptoms such as cognitive changes, dizziness, visual symptoms, fatigue, and functional limitations.

Evaluation of post-traumatic headache patterns (including migraine, occipital neuralgia, cervicogenic/facet-related, myofascial), with targeted treatment planning and coordination with neurology or pain management when needed.

Care for cervical spine and soft tissue injuries after trauma, including stiffness, reduced range of motion, and symptoms of cervical radiculopathy (radiating arm pain, numbness, or weakness) and coordination with pain management when needed.

Comprehensive evaluation and treatment of nerve conditions affecting a single nerve (such as carpal tunnel syndrome), nerve plexus compression or injury (such as thoracic outlet syndrome), or more widespread nerve disorders (such as peripheral neuropathy). Diagnostic evaluation may include nerve testing (EMG/NCS), imaging, laboratory studies, and in some cases skin biopsy to assess small fiber nerve function. Treatment is highly individualized and is based on the severity of symptoms and nerve injury.

Meet Dr. Casey Emigh
Dr. Casey Emigh, M.D. (pronounced “Dr. Emmy”) is a board-certified Physiatrist (Physical Medicine & Rehabilitation/PM&R physician). She completed medical school in Kansas and residency in Washington before relocating to Colorado with her husband and dogs to be closer to family.
After practicing within a large healthcare system in Northern Colorado, she recognized the need for more time, flexibility, and collaborative care when serving patients with complex symptoms and diagnoses. This led to the creation of Foundations Physiatry in 2025.
She has a clinical interest in brain injury and concussion, hypermobility and connective tissue disorders, and a wide range of musculoskeletal and peripheral nerve conditions.
Her work is grounded in listening closely, identifying what is truly driving symptoms, and developing personalized plans that support recovery of both the brain and body—while honoring the whole person.

For a specific concern with a clear starting point
60 minutes — $325
This visit is ideal when you have a defined issue—such as a recent injury, pain in one area, or targeted question—and are looking for clarity and next steps.

For complex or long-standing concerns
90 minutes — $495
Designed for patients with more complex presentations, including overlapping symptoms or conditions affecting multiple areas of life.
This visit allows us to step back, look at the full picture, and build a thoughtful, individualized plan.
Please note, an additional fee may apply for extensive record review. Patients with extensive records who would like to avoid this fee may ask our team about ways to organize and summarize their records before the visit.

Ongoing care and plan refinement
45 minutes — $195
Follow-up visits are used to monitor progress, adjust your plan, and continue building toward meaningful improvement.

For continuity of care or more in-depth reassessment
60 minutes — $195 (limited availability through June 2026)
These visits are recommended in select situations, including care transitions or when additional time is needed for reassessment and planning. We will guide you if this is the right fit.

For nerve- and muscle-related concerns
1 limb — $350
2 limb — $525
3 limb — $700
4 limb — $825
Designed for patients seeking timely evaluation of symptoms such as numbness, tingling, weakness, pain, or suspected nerve injury.
This visit combines physician evaluation with same-day EMG/NCS testing, often allowing diagnosis and treatment planning in a single visit.

For musculoskeletal pain and nerve irritation or injury
Procedure visit:
Dry needling — $95
Trigger point injections — $125
Ultrasound-guided injections — $175
Same-day procedures may be available (depending on timing and complexity) at a reduced add-on rate.
Designed for patients seeking targeted treatment for myofascial pain, nerve irritation, or related musculoskeletal concerns.
These visits focus on symptom relief while also addressing movement patterns, muscle activation, and contributing biomechanical or neurologic factors as appropriate.
Many new patients begin with a Focused Evaluation, while those with more complex or long-standing concerns may benefit from a Comprehensive Evaluation.
